Genes affected
RECQL4 (HGNC:9949): (RecQ like helicase 4) The protein encoded by this gene is a DNA helicase that belongs to the RecQ helicase family. DNA helicases unwind double-stranded DNA into single-stranded DNAs and may modulate chromosome segregation. This gene is predominantly expressed in thymus and testis. Mutations in this gene are associated with Rothmund-Thomson, RAPADILINO and Baller-Gerold syndromes. [provided by RefSeq, Jan 2010]

Our verdict: Likely_benign. The variant received -5 ACMG points.

PP5
Variant 8-144516055-C-T is Pathogenic according to our data. Variant chr8-144516055-C-T is described in ClinVar as Conflicting_classifications_of_pathogenicity. ClinVar VariationId is 407037.
BP4
Computational evidence support a benign effect (MetaRNN=0.021392375). . Strength limited to SUPPORTING due to the PP5.
BS1
Variant frequency is greater than expected in population eas. GnomAdExome4 allele frequency = 0.000336 (491/1460294) while in subpopulation EAS AF = 0.0115 (455/39694). AF 95% confidence interval is 0.0106. There are 4 homozygotes in GnomAdExome4. There are 256 alleles in the male GnomAdExome4 subpopulation. Median coverage is 66. This position passed quality control check. Existence of Clinvar submissions makes me limit the strength of this signal to Supporting

This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 355 of the RECQL4 protein (p.Arg355Gln). This variant is present in population databases (rs374743591, gnomAD 0.2%). This variant has not been reported in the literature in individuals affected with RECQL4-related conditions. ClinVar contains an entry for this variant (Variation ID: 407037). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on RECQL4 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
