Variant summary
Our verdict is Uncertain significance. The variant received 4 ACMG points: 4P and 0B. PM1PM2
The NM_004360.5(CDH1):c.821G>T(p.Gly274Val) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.00000274 in 1,461,298 control chromosomes in the GnomAD database, with no homozygous occurrence. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. G274S) has been classified as Benign.

Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Identified in an individual with concern for hereditary breast and ovarian cancer syndrome (PMID: 36647026); This variant is associated with the following publications: (PMID: 36647026, 15235021, 22850631) -
Hereditary diffuse gastric adenocarcinoma Uncertain:1
In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. ClinVar contains an entry for this variant (Variation ID: 463794). This variant has not been reported in the literature in individuals affected with CDH1-related conditions. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces glycine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 274 of the CDH1 protein (p.Gly274Val). -
Hereditary cancer-predisposing syndrome Uncertain:1
The p.G274V variant (also known as c.821G>T), located in coding exon 6 of the CDH1 gene, results from a G to T substitution at nucleotide position 821. The glycine at codon 274 is replaced by valine, an amino acid with dissimilar properties. This variant was identified in an individual meeting testing criteria for Hereditary Breast and Ovarian Cancer syndrome, but personal and family medical history was not specifically provided (Chrysafi P et al. Cancers (Basel), 2023 Dec;15:). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -
