Arrhythmogenic cardiomyopathy with wooly hair and keratoderma Uncertain:1
This missense variant replaces serine with leucine at codon 1049 of the DSP protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with cardiovascular disorders in the literature. This variant has been identified in 27/282260 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
Lethal acantholytic epidermolysis bullosa Uncertain:1
This variant was observed in the ICSL laboratory as part of a predisposition screen in an ostensibly healthy population. It had not been previously curated by ICSL or reported in the Human Gene Mutation Database (HGMD: prior to June 1st, 2018), and was therefore a candidate for classification through an automated scoring system. Utilizing variant allele frequency, disease prevalence and penetrance estimates, and inheritance mode, an automated score was calculated to assess if this variant is too frequent to cause the disease. Based on the score, this variant could not be ruled out of causing disease and therefore its association with disease required further investigation. A literature search was performed for the gene, cDNA change, and amino acid change (if applicable). No publications were found based on this search. This variant was therefore classified as a variant of unknown significance for this disease. -

not provided Uncertain:1
Identified in a patient with dilated cardiomyopathy diagnosed on fetal echocardiogram; however, this patient also harbored other cardiogenetic variants (PMID: 31770195); Identified in an individual with unexplained cardiac arrest; however, additional clinical information was not provided (PMID: 35352813); In silico analysis indicates that this missense variant does not alter protein structure/function; This variant is associated with the following publications: (PMID: 35352813, 31770195, 30775854) -
Cardiovascular phenotype Uncertain:1
The p.S1049L variant (also known as c.3146C>T), located in coding exon 23 of the DSP gene, results from a C to T substitution at nucleotide position 3146. The serine at codon 1049 is replaced by leucine, an amino acid with dissimilar properties. This alteration has been reported in a hypertrophic cardiomyopathy (HCM) cohort; however, clinical details were limited (Jääskeläinen P et al. ESC Heart Fail, 2019 Apr;6:436-445). Additionally, this alteration was detected in a case report of a fetus with severe tricuspid valve regurgitation, who developed dilated cardiomyopathy (DCM); however, additional alterations in other cardiac-related genes were identified (Liu L et al. Medicine (Baltimore), 2019 Nov;98:e17771). This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
