Genes affected
RAPSN (HGNC:9863): (receptor associated protein of the synapse) This gene encodes a member of a family of proteins that are receptor associated proteins of the synapse. The encoded protein contains a conserved cAMP-dependent protein kinase phosphorylation site, and plays a critical role in clustering and anchoring nicotinic acetylcholine receptors at synaptic sites by linking the receptors to the underlying postsynaptic cytoskeleton, possibly by direct association with actin or spectrin. Mutations in this gene may play a role in postsynaptic congenital myasthenic syndromes. Alternatively spliced transcript variants encoding multiple isoforms have been observed for this gene. ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 16 ACMG points.

PM1
In a hotspot region, there are 3 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 0 benign, 4 uncertain in NM_005055.5
PM2
Very rare variant in population databases, with high coverage;
PM5
Other missense variant is known to change same aminoacid residue: Variant chr11-47447852-C-T is described in ClinVar as Conflicting_classifications_of_pathogenicity. ClinVar VariationId is 1285362.
PP3
MetaRNN computational evidence supports a deleterious effect, 0.871
PP5
Variant 11-47447853-G-C is Pathogenic according to our data. Variant chr11-47447853-G-C is described in ClinVar as Pathogenic/Likely_pathogenic. ClinVar VariationId is 1417647.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.
