Variant summary
Our verdict is Pathogenic. The variant received 18 ACMG points: 18P and 0B. PVS1PM2PP5_Very_Strong
The NM_005359.6(SMAD4):c.1096C>T(p.Gln366*) variant causes a stop gained change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★★). Synonymous variant affecting the same amino acid position (i.e. Q366Q) has been classified as Likely benign. Variant results in nonsense mediated mRNA decay.

The p.Q366* pathogenic mutation (also known as c.1096C>T), located in coding exon 8 of the SMAD4 gene, results from a C to T substitution at nucleotide position 1096. This changes the amino acid from a glutamine to a stop codon within coding exon 8. This mutation has been reported in an individual diagnosed with juvenile polyposis syndrome and/or hereditary hemorrhagic telangiectasia (Wain et al. Genet. Med. 2014;16(8):588-93). In addition to the clinical data presented in the literature, since premature stop codons are typically deleterious in nature, this alteration is interpreted as a disease-causing mutation (ACMG Recommendations for Standards for Interpretation and Reporting of Sequence Variations. Revision 2007. Genet Med. 2008;10:294). -
Juvenile polyposis syndrome Pathogenic:1
This sequence change creates a premature translational stop signal (p.Gln366*) in the SMAD4 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in SMAD4 are known to be pathogenic (PMID: 16152648, 16436638, 22810475). This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with SMAD4-related conditions. ClinVar contains an entry for this variant (Variation ID: 405490). For these reasons, this variant has been classified as Pathogenic. -
