Genes affected
HAX1 (HGNC:16915): (HCLS1 associated protein X-1) The protein encoded by this gene is known to associate with hematopoietic cell-specific Lyn substrate 1, a substrate of Src family tyrosine kinases. It also interacts with the product of the polycystic kidney disease 2 gene, mutations in which are associated with autosomal-dominant polycystic kidney disease, and with the F-actin-binding protein, cortactin. It was earlier thought that this gene product is mainly localized in the mitochondria, however, recent studies indicate it to be localized in the cell body. Mutations in this gene result in autosomal recessive severe congenital neutropenia, also known as Kostmann disease. Two transcript variants encoding different isoforms have been found for this gene. ACMG classification

Classification was made for transcript

Our verdict: Likely_benign. The variant received -3 ACMG points.

BP4
Computational evidence support a benign effect (MetaRNN=0.11071029).
BS1
Variant frequency is greater than expected in population mid. GnomAdExome4 allele frequency = 0.000118 (173/1461888) while in subpopulation MID AF = 0.00537 (31/5768). AF 95% confidence interval is 0.00389. There are 0 homozygotes in GnomAdExome4. There are 105 alleles in the male GnomAdExome4 subpopulation. Median coverage is 33. This position passed quality control check. Existence of Clinvar submissions makes me limit the strength of this signal to Supporting

This sequence change replaces glycine, which is neutral and non-polar, with alanine, which is neutral and non-polar, at codon 143 of the HAX1 protein (p.Gly143Ala). This variant is present in population databases (rs755031266, gnomAD 0.07%). This missense change has been observed in individual(s) with severe congenital neutropenia (PMID: 19036076). ClinVar contains an entry for this variant (Variation ID: 574945). Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

The c.428G>C (p.G143A) alteration is located in exon 3 (coding exon 3) of the HAX1 gene. This alteration results from a G to C substitution at nucleotide position 428, causing the glycine (G) at amino acid position 143 to be replaced by an alanine (A). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear.
