NM_006514.4:c.4164G>A
Variant summary
Our verdict is Likely benign. Variant got -4 ACMG points: 0P and 4B. BS2
The NM_006514.4(SCN10A):c.4164G>A(p.Trp1388*) variant causes a stop gained change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000156 in 1,606,000 control chromosomes in the GnomAD database, including 1 homozygotes. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Variant results in nonsense mediated mRNA decay.

Brugada syndrome Uncertain:1
This sequence change creates a premature translational stop signal (p.Trp1388*) in the SCN10A gene. It is expected to result in an absent or disrupted protein product. However, the current clinical and genetic evidence is not sufficient to establish whether loss-of-function variants in SCN10A cause disease. This variant is present in population databases (rs756253735, gnomAD 0.002%). This variant has not been reported in the literature in individuals affected with SCN10A-related conditions. ClinVar contains an entry for this variant (Variation ID: 463258). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
not provided Uncertain:1
The W1388X variant in the SCN10A gene has not been reported previously as a pathogenic variant nor as a benign variant, to our knowledge. This variant is predicted to cause loss of normal protein function either through protein truncation or nonsense-mediated mRNA decay. The W1388X variant was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. We interpret W1388X as a variant of uncertain significance. -
Cardiovascular phenotype Uncertain:1
The p.W1388* variant (also known as c.4164G>A), located in coding exon 24 of the SCN10A gene, results from a G to A substitution at nucleotide position 4164. This changes the amino acid from a tryptophan to a stop codon within coding exon 24. This alteration is expected to result in premature protein truncation or nonsense-mediated mRNA decay. However, loss of function of SCN10A has not been clearly established as a mechanism of disease. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
