BRCA1 (HGNC:1100): (BRCA1 DNA repair associated) This gene encodes a 190 kD nuclear phosphoprotein that plays a role in maintaining genomic stability, and it also acts as a tumor suppressor. The BRCA1 gene contains 22 exons spanning about 110 kb of DNA. The encoded protein combines with other tumor suppressors, DNA damage sensors, and signal transducers to form a large multi-subunit protein complex known as the BRCA1-associated genome surveillance complex (BASC). This gene product associates with RNA polymerase II, and through the C-terminal domain, also interacts with histone deacetylase complexes. This protein thus plays a role in transcription, DNA repair of double-stranded breaks, and recombination. Mutations in this gene are responsible for approximately 40% of inherited breast cancers and more than 80% of inherited breast and ovarian cancers. Alternative splicing plays a role in modulating the subcellular localization and physiological function of this gene. Many alternatively spliced transcript variants, some of which are disease-associated mutations, have been described for this gene, but the full-length natures of only some of these variants has been described. The BRCA1 c.1802A>G (p.His601Arg) variant has been reported in the published literature in a family where three sisters that carried the variant did not have a personal history of cancer, and two sisters that did not carry the variant were affected with breast cancer (PMID: 15726418 (2005)). This variant has also been reported to be located in a region of the BRCA1 gene that tolerant to missense changes (PMID: 31911673 (2020)). The frequency of this variant in the general population, 0.00012 (3/24892 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded conflicting predictions that this variant is benign or damaging. Based on the available information, we are unable to determine the clinical significance of this variant. -

The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.1802A>G has been reported in the literature in 3 unaffected African American sisters and was not present in their 2 affected sisters with Breast Cancer (McKean-Cowdin_2005) and was not observed to segregate with disease. This report does not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported.
