The p.H662Y variant (also known as c.1984C>T), located in coding exon 9 of the BRCA1 gene, results from a C to T substitution at nucleotide position 1984. The histidine at codon 662 is replaced by tyrosine, an amino acid with similar properties. This alteration was observed in an individual diagnosed with breast cancer before the age of 40 (Loizidou M et al. Clin Genet. 2007; 71:165-70). This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.

Variant summary: BRCA1 c.1984C>T (p.His662Tyr) results in a conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant was absent in 251084 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.1984C>T has been reported in the literature in individuals affected with Breast Cancer (Loizidou_2007, Loizidou_2017, Dorling_2021). These reports do not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Two ClinVar submitters have assessed the variant since 2014: both classified the variant as of uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance.

This sequence change replaces histidine, which is basic and polar, with tyrosine, which is neutral and polar, at codon 662 of the BRCA1 protein (p.His662Tyr). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with breast cancer (PMID: 17250666, 27882536). ClinVar contains an entry for this variant (Variation ID: 54426). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt BRCA1 protein function with a negative predictive value of 95%. RNA analysis performed to evaluate the impact of this missense change on mRNA splicing indicates it does not significantly alter splicing (Invitae). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
