The BRCA1 c.2123C>T; p.Ser708Phe variant (rs80357182), to our knowledge, is not reported in the medical literature but is reported in ClinVar (Variation ID: 91576). This variant is absent from the Genome Aggregation Database, indicating it is not a common polymorphism. The serine at codon 708 is weakly conserved, but computational analyses are uncertain whether this variant is neutral or deleterious (REVEL: 0.226). A different missense variant at the same serine codon (p.Ser708Tyr) is shown to be non-pathogenic by in vitro functional assays (Bouwman 2020), although it is classified by most sources as a variant of uncertain significance in ClinVar (Variation ID: 37447). In a large-scale breast cancer association study, the variant was observed in an individual with breast cancer as well as in an unaffected individual (PMID: 33471991 (2021), see also LOVD (http://databases.lovd.nl/shared/genes/BRCA1)). This variant has also been reported to be located in a region of the BRCA1 gene that is tolerable to missense changes (PMID: 31911673 (2020)). This variant has not been reported in large, multi-ethnic general populations (Genome Aggregation Database, http://gnomad.broadinstitute.org). Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant. -

Variant summary: BRCA1 c.2123C>T (p.Ser708Phe) results in a non-conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant was absent in 251170 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.2123C>T in individuals affected with Hereditary Breast And Ovarian Cancer Syndrome and no experimental evidence demonstrating its impact on protein function have been reported. Eight submitters have provided clinical-significance assessments for this variant to ClinVar after 2014 and classified the variant as either VUS (n=5) or likely benign (n=3). Based on the evidence outlined above, the variant was classified as uncertain significance. -

At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -

This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with BRCA1-related conditions. ClinVar contains an entry for this variant (Variation ID: 91576). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt BRCA1 protein function with a negative predictive value of 80%.
