DNA sequence analysis of the BRCA1 gene demonstrated a sequence change, c.3711A>G, in exon 10 that results in an amino acid change, p.Ile1237Met. This sequence change has been previously described in individuals with personal and/or family history of breast and/or ovarian cancer, colorectal cancer and papillary serous carcinoma of the peritoneum and (PMID: 10728699, 24884479, 25896959, 27062684). This sequence change has been described in the gnomAD database in one individual which corresponds to a population frequency of 0.0004% (dbSNP rs80357388). The p.Ile1237Met change affects a poorly conserved amino acid residue located in a domain of the BRCA1 protein that is known to be functional. The p.Ile1237Met substitution appears to be benign using several in-silico pathogenicity prediction tools (SIFT, PolyPhen2, Align GVGD, REVEL). Due to insufficient evidences and the lack of functional studies, the clinical significance of the p.Ile1237Met change remains unknown at this time. -
Variant summary: BRCA1 c.3711A>G (p.Ile1237Met) results in a conservative amino acid change in the encoded protein sequence. Five of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 251284 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.3711A>G has been reported in the literature as a VUS in settings of individuals affected with and/or undergoing testing for a variety of cancers (example, Dargenio_2015, Schorge_2000, Silva_2014, Azzollini_2016, Fountzilas_2018). These report(s) do not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. At least one publication reports experimental evidence evaluating an impact on protein function (Bouwman_2020). These results showed no damaging effect of this variant on ability to complement BRCA1-deficient mouse embryonic stem cells in homologous recombination DNA repair (HRR) using cisplatin and olaparib sensitivity assays and a direct GFP HRR assay. Multiple laboratories reported the variant with conflicting assessments (likely benign, n=2; VUS, n=5). Based on the evidence outlined above, the variant was classified as likely benign. -

This variant was observed in the ICSL laboratory as part of a predisposition screen in an ostensibly healthy population. It had not been previously curated by ICSL or reported in the Human Gene Mutation Database (HGMD: prior to June 1st, 2018), and was therefore a candidate for classification through an automated scoring system. Utilizing variant allele frequency, disease prevalence and penetrance estimates, and inheritance mode, an automated score was calculated to assess if this variant is too frequent to cause the disease. Based on the score, this variant could not be ruled out of causing disease and therefore its association with disease required further investigation. A literature search was performed for the gene, cDNA change, and amino acid change (if applicable). No publications were found based on this search. This variant was therefore classified as a variant of unknown significance for this disease. -

The p.Ile1237Met variant was not identified in the literature. The variant was identified in dbSNP (ID: rs80357388) “With Uncertain significance allele”, the ClinVar database (classified with uncertain significance by Ambry Genetics), the BIC database (1X with unknown clinical importance), and UMD (1X as an unclassified variant).The variant occurs outside of the splicing consensus sequence and in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer, HumanSpliceFinder) do not predict a difference in splicing. However, this information is not predictive enough to rule out pathogenicity. The p.Ile1237 residue is not conserved in mammals and the variant methionine (Met) is present in dog, increasing the likelihood this variant doesn't have clinical significance. Computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) do not suggest a high likelihood of impact to the protein. This sequence change replaces isoleucine, which is neutral and non-polar, with methionine, which is neutral and non-polar, at codon 1237 of the BRCA1 protein (p.Ile1237Met). This variant is present in population databases (rs80357388, gnomAD 0.0009%). This missense change has been observed in individual(s) with papillary serous carcinoma of the peritoneum and personal and/or family history of breast and/or ovarian cancer (PMID: 10728699, 24884479, 25896959, 27062684). This variant is also known as 3830A>G. ClinVar contains an entry for this variant (Variation ID: 54973). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt BRCA1 protein function with a negative predictive value of 80%. Experimental studies have shown that this missense change does not substantially affect BRCA1 function (PMID: 32546644). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
