Variant summary
Our verdict is Likely pathogenic. The variant received 6 ACMG points: 6P and 0B. PM2PP3_Strong
The NM_013382.7(POMT2):c.1711C>T(p.Pro571Ser) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.0000356 in 1,461,894 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. 13/22 in silico tools predict a damaging outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Synonymous variant affecting the same amino acid position (i.e. P571P) has been classified as Likely benign.

Not observed at a significant frequency in large population cohorts (Lek et al., 2016); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Has not been previously published as pathogenic or benign to our knowledge -

Variant summary: POMT2 c.1711C>T (p.Pro571Ser) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant allele was found at a frequency of 2.4e-05 in 251472 control chromosomes. c.1711C>T has been observed in individual(s) affected with Limb-Girdle Muscular Dystrophy, Autosomal Recessive (Marchant_2024, LCG internal data). These data indicate that the variant may be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 38544359). ClinVar contains an entry for this variant (Variation ID: 284056). Based on the evidence outlined above, the variant was classified as VUS-possibly pathogenic. -

This sequence change replaces proline, which is neutral and non-polar, with serine, which is neutral and polar, at codon 571 of the POMT2 protein (p.Pro571Ser). This variant is present in population databases (rs771812476, gnomAD 0.004%). This missense change has been observed in individual(s) with clinical features of POMT2-related conditions (Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 284056). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on POMT2 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
Myopathy caused by variation in POMT2 Uncertain:1
The p.Pro571Ser variant in POMT2 was identified by our study, in the compound heterozygous state along with another variant of uncertain significance, in 1 individual with myopathy. The phase of these variants are unknown at this time. The p.Pro571Ser variant in POMT2 has been reported in 1 individual with limb girdle muscular dystrophy (PMID: 38544359), and has been identified in 0.004% (50/1180028) of European (non-Finnish) chromosomes by the Genome Aggregation Database (gnomAD, http://gnomad.broadinstitute.org; dbSNP rs771812476). Although this variant has been seen in the general population in a heterozygous state, its frequency is low enough to be consistent with a recessive carrier frequency. This variant has also been reported in ClinVar (Variation ID: 284056) and has been interpreted as a variant of uncertain significance by Eurofins Ntd Llc, Labcorp Genetics, GeneDx, and Revvity Omics. Of the 2 affected individuals, 1 was a compound heterozygote that carried a reported pathogenic variant in trans, which increases the likelihood that the p.Pro571Ser variant is pathogenic (Variation ID: 3221; PMID: 38544359). Computational prediction tools and conservation analyses suggest that this variant may impact the protein, though this information is not predictive enough to determine pathogenicity. In summary, the clinical significance of the p.Pro571Ser variant is uncertain. ACMG/AMP Criteria applied: PP3_moderate, PM2_supporting, PM3_supporting (Richards 2015). -
