POT1 (HGNC:17284): (protection of telomeres 1) This gene is a member of the telombin family and encodes a nuclear protein involved in telomere maintenance. Specifically, this protein functions as a member of a multi-protein complex that binds to the TTAGGG repeats of telomeres, regulating telomere length and protecting chromosome ends from illegitimate recombination, catastrophic chromosome instability, and abnormal chromosome segregation. Increased transcriptional expression of this gene is associated with stomach carcinogenesis and its progression. Alternatively spliced transcript variants have been described. This sequence change replaces serine, which is neutral and polar, with arginine, which is basic and polar, at codon 472 of the POT1 protein (p.Ser472Arg). This variant is present in population databases (rs146643991, gnomAD 0.02%). This variant has not been reported in the literature in individuals affected with POT1-related conditions. ClinVar contains an entry for this variant (Variation ID: 475044). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt POT1 protein function with a negative predictive value of 80%. RNA analysis performed to evaluate the impact of this missense change on mRNA splicing indicates it does not significantly alter splicing (internal data). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

DNA sequence analysis of the POT1 gene demonstrated a sequence change, c.1416T>G, in exon 15 that results in an amino acid change, p.Ser472Arg. This sequence change does not appear to have been previously described in patients with POT1-related disorders. This sequence change has been described in the gnomAD database with a low population frequency of 0.009% (dbSNP rs146643991). The p.Ser472Arg change affects a moderately conserved amino acid residue of the POT1 protein. In-silico pathogenicity prediction tools (SIFT, PolyPhen2, Align GVGD, REVEL) provide contradictory results for the p.Ser472Arg substitution. Due to these contrasting evidences and the lack of functional studies, the clinical significance of the p.Ser472Arg change remains unknown at this time. -

To the best of our knowledge, the variant has not been reported in the published literature. The frequency of this variant in the general population, 0.00019 (24/129126 chromosomes in European (Non-Finnish) subpopulation, http://gnomad.broadinstitute.org), is higher than would generally be expected for pathogenic variants in this gene. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is damaging. Additional analysis using software algorithms for the prediction of the effect of nucleotide changes on POT1 mRNA splicing yielded inconclusive findings . Based on the available information, we are unable to determine the clinical significance of this variant. -

The p.S472R variant (also known as c.1416T>G), located in coding exon 11 of the POT1 gene, results from a T to G substitution at nucleotide position 1416. The serine at codon 472 is replaced by arginine, an amino acid with dissimilar properties. This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -
