NM_017617.5:c.2359A>G
Variant summary
Our verdict is Likely benign. The variant received -4 ACMG points: 1P and 5B. PP3BS1_SupportingBS2
The NM_017617.5(NOTCH1):c.2359A>G(p.Asn787Asp) variant causes a missense change. The variant allele was found at a frequency of 0.0000167 in 1,612,612 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. N787H) has been classified as Likely benign.

Has not been previously published as pathogenic or benign to our knowledge; In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Not observed at significant frequency in large population cohorts (gnomAD) -
Familial thoracic aortic aneurysm and aortic dissection Uncertain:1
The c.2359A>G (p.N787D) alteration is located in exon 15 (coding exon 15) of the NOTCH1 gene. This alteration results from a A to G substitution at nucleotide position 2359, causing the asparagine (N) at amino acid position 787 to be replaced by an aspartic acid (D). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear. -
Adams-Oliver syndrome 5 Uncertain:1
This sequence change replaces asparagine, which is neutral and polar, with aspartic acid, which is acidic and polar, at codon 787 of the NOTCH1 protein (p.Asn787Asp). The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This variant has not been reported in the literature in individuals affected with NOTCH1-related conditions. ClinVar contains an entry for this variant (Variation ID: 544164). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt NOTCH1 protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
