TRMU (HGNC:25481): (tRNA mitochondrial 2-thiouridylase) This nuclear gene encodes a mitochondrial tRNA-modifying enzyme. The encoded protein catalyzes the 2-thiolation of uridine on the wobble positions of tRNA(Lys), tRNA(Glu), and tRNA(Gln), resulting in the formation of 5-taurinomethyl-2-thiouridine moieties. Mutations in this gene may cause transient infantile liver failure. Polymorphisms in this gene may also influence the severity of deafness caused by mitochondrial 12S ribosomal RNA mutations. This sequence change affects the initiator methionine of the TRMU mRNA. The next in-frame methionine is located at codon 36. This variant is not present in population databases (gnomAD no frequency). Disruption of the initiator codon has been observed in individuals with TRMU-related conditions (PMID: 19732863, 31395954, 33365252). ClinVar contains an entry for this variant (Variation ID: 280795). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -

The c.2 T>C pathogenic variant in the TRMU gene has not been reported previously as a pathogenic variant nor as a benign variant, to our knowledge. The c.2 T>C variant was not observed in approximately 6500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. As this pathogenic variant changes the translation initiator Methionine codon, the resultant protein is described as p.Met1?, using a question mark to signify that it is not known if the loss of Met1 means that all protein translation is completely prevented or if an abnormal protein is produced using an alternate Methionine. We interpret c.2 T>C as a pathogenic variant. -
