NM_018060.4:c.2726_2727delCTinsTC

Variant summary

Our verdict is Uncertain significance. The variant received 5 ACMG points: 5P and 0B. PS1_ModeratePM5PP3

The NM_018060.4(IARS2):c.2726_2727delCTinsTC(p.Pro909Leu) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. It is difficult to determine the true allele frequency of this variant because it is of type MNV, and the frequency of such variant types in population databases may be underestimated and unreliable. No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar. Another nucleotide change resulting in the same amino acid substitution has been previously reported as Likely pathogenic in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. P909S) has been classified as Pathogenic.

Genes affected
IARS2 (HGNC:29685): (isoleucyl-tRNA synthetase 2, mitochondrial) Aminoacyl-tRNA synthetases catalyze the aminoacylation of tRNA by their cognate amino acid. Because of their central role in linking amino acids with nucleotide triplets contained in tRNAS, aminoacyl-tRNA synthetases are thought to be among the first proteins that appeared in evolution. Two forms of isoleucine-tRNA synthetase exist, a cytoplasmic form and a mitochondrial form. This gene encodes the mitochondrial isoleucine-tRNA synthetase which belongs to the class-I aminoacyl-tRNA synthetase family. [provided by RefSeq, Dec 2014]

ACMG classification

Classification was made for transcript

Our verdict: Uncertain_significance. The variant received 5 ACMG points.

PS1
Transcript NM_018060.4 (IARS2) is affected with MISSENSE_VARIANT having same AA change as one Pathogenic present in ClinVar.
PM5
Other missense variant is known to change same aminoacid residue: Variant chr1-220143108-C-T is described in ClinVar as Pathogenic. ClinVar VariationId is 638571.Status of the report is no_assertion_criteria_provided, 0 stars.
PP3
No computational evidence supports a deleterious effect, but strongly conserved according to phyloP
