Genes affected
NSD1 (HGNC:14234): (nuclear receptor binding SET domain protein 1) This gene encodes a protein containing a SET domain, 2 LXXLL motifs, 3 nuclear translocation signals (NLSs), 4 plant homeodomain (PHD) finger regions, and a proline-rich region. The encoded protein enhances androgen receptor (AR) transactivation, and this enhancement can be increased further in the presence of other androgen receptor associated coregulators. This protein may act as a nucleus-localized, basic transcriptional factor and also as a bifunctional transcriptional regulator. Mutations of this gene have been associated with Sotos syndrome and Weaver syndrome. One version of childhood acute myeloid leukemia is the result of a cryptic translocation with the breakpoints occurring within nuclear receptor-binding Su-var, enhancer of zeste, and trithorax domain protein 1 on chromosome 5 and nucleoporin, 98-kd on chromosome 11. Multiple transcript variants encoding distinct isoforms have been identified for this gene. [provided by RefSeq, Sep 2018]

ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 18 ACMG points.

PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant is located in the 3'-most 50 bp of the penultimate exon, not predicted to undergo nonsense mediated mRNA decay. There are 88 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 5-177292131-T-TG is Pathogenic according to our data. Variant chr5-177292131-T-TG is described in ClinVar as Pathogenic/Likely_pathogenic. ClinVar VariationId is 547727.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.

For these reasons, this variant has been classified as Pathogenic. This variant disrupts the C-terminus of the NSD1 protein. Other variant(s) that disrupt this region (p.Glu2564*) have been determined to be pathogenic (Invitae). This suggests that variants that disrupt this region of the protein are likely to be causative of disease. This variant has not been reported in the literature in individuals with NSD1-related conditions. ClinVar contains an entry for this variant (Variation ID: 547727). This variant is not present in population databases (ExAC no frequency). This sequence change results in a premature translational stop signal in the NSD1 gene (p.Cys2146Trpfs*20). While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 551 amino acids of the NSD1 protein. -
