This variant is denoted PALB2 c.2326T>C at the cDNA level, p.Phe776Leu (F776L) at the protein level, and results in the change of a Phenylalanine to a Leucine (TTC>CTC). This variant has not, to our knowledge, been published in the literature as pathogenic or benign. PALB2 Phe776Leu was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. Since Phenylalanine and Leucine share similar properties, this is considered a conservative amino acid substitution. PALB2 Phe776Leu occurs at a position that is not conserved and is located in the region required for interaction with POLH and POLH DNA synthesis stimulation (UniProt). In silico analyses predict that this variant is unlikely to alter protein structure or function. Based on currently available information, it is unclear whether PALB2 Phe776Leu is pathogenic or benign. We consider it to be a variant of uncertain significance. -
In the published literature, this variant has been reported in an individual with breast cancer (PMID: 17200668 (2007)). The frequency of this variant in the general population, 0.000004 (1/251460 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. The p.F776L variant (also known as c.2326T>C), located in coding exon 5 of the PALB2 gene, results from a T to C substitution at nucleotide position 2326. The phenylalanine at codon 776 is replaced by leucine, an amino acid with highly similar properties. In one study, this alteration was identified in 1/923 familial breast cancer cases and 0/1084 controls (Rahman N et al. Nat Genet, 2007 Feb;39:165-7). This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
