The p.M1? variant (also known as c.2T>C) is located in coding exon 1 of the PALB2 gene and results from a T to C substitution at nucleotide position 2. This alters the methionine residue at the initiation codon (ATG). This alteration has been identified in multiple individuals with a personal and/or family history of breast and/or ovarian cancer (Cybulski C et al. Int J Cancer 2019 12;145(12):3311-3320; Dorling L et al. N Engl J Med 2021 02;384(5):428-439; Espinel W et al. Cancers (Basel), 2022 May;14:). This amino acid position is highly conserved in available vertebrate species. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). In addition to the clinical data presented in the literature, sequence variations that modify the initiation codon are expected to result in either loss of translation initiation, N-terminal truncation, or cause a shift in the mRNA reading frame. Based on the majority of available evidence to date, this variant is likely to be pathogenic. -

This sequence change affects the initiator methionine of the PALB2 mRNA. The next in-frame methionine is located at codon 296. This variant is present in population databases (rs756519825, gnomAD 0.0009%). Disruption of the initiator codon has been observed in individual(s) with breast cancer, pancreatic cancer and bile duct cancer (PMID: 31173646, 31263571, 31871297). ClinVar contains an entry for this variant (Variation ID: 241558). If translation initiation is rescued by the downstream methionine at codon 296, this would result in loss of the coiled-coil domain (p.Leu9-Glu42) of the PALB2 protein, which is critical for the interaction between BRCA1/BRCA2, and necessary for homology-directed DNA repair (PMID: 16793542, 19369211, 26649820, 25099575). This variant disrupts a region of the PALB2 protein in which other variant(s) (p.Leu35Pro) have been determined to be pathogenic (PMID: 28319063, 30337689, 31586400). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic. -
