Variant summary: PALB2 c.712A>G (p.Arg238Gly) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change. The variant allele was found at a frequency of 1.6e-05 in 251128 control chromosomes, predominantly at a frequency of 0.00012 within the Latino subpopulation in the gnomAD database. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.712A>G has been observed in one individual affected with BRCA-negative breast cancer and one unaffected control subject (Weitzel_2019). These report(s) do not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication has been ascertained in the context of this evaluation (PMID: 31206626). ClinVar contains an entry for this variant (Variation ID: 233268). The p.R238G variant (also known as c.712A>G), located in coding exon 4 of the PALB2 gene, results from an A to G substitution at nucleotide position 712. The arginine at codon 238 is replaced by glycine, an amino acid with dissimilar properties. This amino acid position is not well conserved on limited sequence alignment. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.

This sequence change replaces arginine, which is basic and polar, with glycine, which is neutral and non-polar, at codon 238 of the PALB2 protein (p.Arg238Gly). ClinVar contains an entry for this variant (Variation ID: 233268). An algorithm developed to predict the effect of missense changes on protein structure and function outputs the following: PolyPhen-2: "Benign". The glycine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function.
