Variant summary
Our verdict is Likely benign. The variant received -2 ACMG points: 0P and 2B. BP4_Moderate
The NM_030813.6(CLPB):c.1949G>T(p.Arg650Leu) variant causes a missense change. The variant allele was found at a frequency of 0.0000161 in 1,614,106 control chromosomes in the GnomAD database, including 1 homozygotes. In-silico tool predicts a benign outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R650C) has been classified as Uncertain significance.

Inborn genetic diseases Uncertain:1
The c.1949G>T (p.R650L) alteration is located in exon 17 (coding exon 17) of the CLPB gene. This alteration results from a G to T substitution at nucleotide position 1949, causing the arginine (R) at amino acid position 650 to be replaced by a leucine (L). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear. -
3-methylglutaconic aciduria, type VIIB Uncertain:1
This sequence change replaces arginine, which is basic and polar, with leucine, which is neutral and non-polar, at codon 650 of the CLPB protein (p.Arg650Leu). This variant is present in population databases (rs137882645, gnomAD 0.003%). This variant has not been reported in the literature in individuals affected with CLPB-related conditions. ClinVar contains an entry for this variant (Variation ID: 476194). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
