Variant summary
Our verdict is Pathogenic. The variant received 11 ACMG points: 11P and 0B. PM1PM2PM5PP3_StrongPP5
The NM_058216.3(RAD51C):c.502A>G(p.Arg168Gly) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.00000205 in 1,461,872 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R168S) has been classified as Likely pathogenic.

Breast-ovarian cancer, familial, susceptibility to, 3 Pathogenic:1
This variant is considered likely pathogenic. Functional studies indicate this variant impacts protein function [PMID: 39299233, 37253112]. This variant is expected to disrupt protein structure [Myriad internal data]. -
Hereditary cancer-predisposing syndrome Uncertain:1
The p.R168G variant (also known as c.502A>G), located in coding exon 3 of the RAD51C gene, results from an A to G substitution at nucleotide position 502. The arginine at codon 168 is replaced by glycine, an amino acid with dissimilar properties. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
Fanconi anemia complementation group O Uncertain:1
This sequence change replaces arginine, which is basic and polar, with glycine, which is neutral and non-polar, at codon 168 of the RAD51C protein (p.Arg168Gly). This variant is present in population databases (rs587781490, gnomAD 0.003%). This variant has not been reported in the literature in individuals affected with RAD51C-related conditions. ClinVar contains an entry for this variant (Variation ID: 825386). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt RAD51C protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
