Variant summary

Our verdict is Likely pathogenic. The variant received 6 ACMG points: 6P and 0B. PS3PP3PP5

The NM_058216.3(RAD51C):c.571+5G>A variant causes a splice region, intron change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.00000558 in 1,613,882 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 3/3 splice prediction tools predicting alterations to normal splicing. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). ClinVar reports functional evidence for this variant: "SCV000255190: Studies have shown that this variant alters mRNA splicing and is expected to lead to the loss of protein expression (PMID:33333735" and additional evidence is available in ClinVar.

Genes affected
RAD51C (HGNC:9820): (RAD51 paralog C) This gene is a member of the RAD51 family. RAD51 family members are highly similar to bacterial RecA and Saccharomyces cerevisiae Rad51 and are known to be involved in the homologous recombination and repair of DNA. This protein can interact with other RAD51 paralogs and is reported to be important for Holliday junction resolution. Mutations in this gene are associated with Fanconi anemia-like syndrome. This gene is one of four localized to a region of chromosome 17q23 where amplification occurs frequently in breast tumors. Overexpression of the four genes during amplification has been observed and suggests a possible role in tumor progression. Alternative splicing results in multiple transcript variants. ACMG classification

Classification was made for transcript

Our verdict: Likely_pathogenic. The variant received 6 ACMG points.

PS3
PS3 evidence extracted from ClinVar submissions: SCV000255190: Studies have shown that this variant alters mRNA splicing and is expected to lead to the loss of protein expression (PMID: 33333735; internal data).; SCV005422733: The variant allele was found at a frequency of 1.6e-05 in 251446 control chromosomes (gnomAD). c.571+5G>A has been reported in the literature in at least an individual affected with Fanconi Anemia as well as in individuals with breast and/or ovarian cancers (example: Jacquinet_2018, Pang_2011, Chan_2018, Yao_2022). The following publications have been ascertained in the context of this evaluation (PMID: 30093976, 29278735, 21597919, 33333735, 35186721). no; SCV000691251: A mini-gene splice assay has shown that this variant results in out-of-frame skipping of exon 3 in over 90% of the transcripts (PMID: 33333735).; SCV005416754: "Well-established in vitro or in vivo functional studies supportive of a damaging effect on the gene or gene product."; SCV005623327: Analysis of this variant using software algorithms for the prediction of the effect of nucleotide changes on splicing yielded predictions that this variant may affect proper RAD51C mRNA splicing. PMID:33333735 Since the instruction specifies that in silico predictions or computational evidence do not count as functional studies, the correct response should adhere strictly to the provided guidelines. Given the context, the output should reflect whether there is actual functional study evidence or not. Therefore, based on the input provided, which does not contain direct evidence of functional studies but rather computational predictions, the appropriate response is: `no`; SCV005897450: Functional studies indicate this variant impacts protein function [PMID: 35740625, 39299233].; SCV006324397: Studies have shown that this variant alters mRNA splicing and is expected to lead to the loss of protein expression., PS3 (supporting pathogenic): Olvera-León (2024, PMID: 39299233): functional_classification: fast depleted
PP3
