This missense variant replaces arginine with histidine at codon 388 of the lamin A/C proteins. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). Functional studies using transfected COS7 cells suggest that this variant may impact LMNA protein activity (PMID: 20160190); however the clinical relevance of this observation is not known. This variant has been reported in a family affected with dilated cardiomyopathy, although the variant did not segregate with disease in multiple affected family members and was present in multiple unaffected family members (PMID: 18585512, 30012837). This variant has been identified in 3/248352 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
Charcot-Marie-Tooth disease type 2 Pathogenic:1
In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. This sequence change replaces arginine, which is basic and polar, with histidine, which is basic and polar, at codon 388 of the LMNA protein (p.Arg388His). This variant is present in population databases (rs267607576, gnomAD 0.006%). This missense change has been observed in individual(s) with dilated cardiomyopathy (PMID: 30012837). ClinVar contains an entry for this variant (Variation ID: 66790). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt LMNA protein function. Experimental studies have shown that this missense change affects LMNA function (PMID: 20160190). This variant disrupts the p.Arg388 amino acid residue in LMNA. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 28125586). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. -
Cardiovascular phenotype Uncertain:1
The p.R388H variant (also known as c.1163G>A), located in coding exon 7 of the LMNA gene, results from a G to A substitution at nucleotide position 1163. The arginine at codon 388 is replaced by histidine, an amino acid with highly similar properties. This alteration has been reported in a family with dilated cardiomyopathy (DCM); however, it did not segregate with disease in that family and an additional alteration in RBM20 was also identified (Parks SB et al. Am Heart J, 2008 Jul;156:161-9; Li D et al. Clin Transl Sci, 2010 Jun;3:90-7). Additionally, this alteration may impact protein function; however, the physiological relevance of this finding is unclear (Cowan J et al. Circ Cardiovasc Genet, 2010 Feb;3:6-14). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
