Variant summary
Our verdict is Likely benign. The variant received -2 ACMG points: 0P and 2B. BP4_Moderate
The NM_198525.3(KIF7):c.2105G>A(p.Arg702Gln) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.000297 in 1,605,938 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R702W) has been classified as Uncertain significance.

Observed in a patient with acrocallosal syndrome in published literature (Putoux et al., 2011); however, additional details were not provided; Published functional studies indicate that R702Q is may represent a hypomorphic allele (Putoux et al., 2011); In silico analysis supports that this missense variant does not alter protein structure/function; This variant is associated with the following publications: (PMID: 21552264)
Acrocallosal syndrome;C1846722:Multiple epiphyseal dysplasia, Al-Gazali type;C3279899:Hydrolethalus syndrome 2 Uncertain:1
Acrocallosal syndrome Uncertain:1
This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 702 of the KIF7 protein (p.Arg702Gln). This variant is present in population databases (rs149078926, gnomAD 0.09%). This missense change has been observed in individual(s) with acrocallosal syndrome (PMID: 21552264). ClinVar contains an entry for this variant (Variation ID: 286561). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt KIF7 protein function. Experimental studies have shown that this missense change affects KIF7 function (PMID: 21552264). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
Multiple epiphyseal dysplasia, Al-Gazali type Uncertain:1
This variant was determined to be of uncertain significance according to ACMG Guidelines, 2015 [PMID:25741868].
