Variant summary
Our verdict is Uncertain significance. Variant got 1 ACMG points: 2P and 1B. PM2BP4
The NM_199242.3(UNC13D):c.856C>T(p.Arg286Trp) variant causes a missense, splice region change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000229 in 1,613,954 control chromosomes in the GnomAD database, including 1 homozygotes. In-silico tool predicts a benign outcome for this variant. 2/3 splice prediction tools predict no significant impact on normal splicing. Variant has been reported in ClinVar as Uncertain significance (★★).

Variant summary: UNC13D c.856C>T (p.Arg286Trp) results in a non-conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. Consensus agreement among computation tools predict no significant impact on normal splicing. However, these predictions have yet to be confirmed by functional studies. The variant allele was found at a frequency of 6e-05 in 250338 control chromosomes. This frequency is not significantly higher than estimated for a pathogenic variant in UNC13D causing Familial Hemophagocytic Lymphohistiocytosis (6e-05 vs 0.0027), allowing no conclusion about variant significance. c.856C>T has been reported in the literature as comound heterozygous in one individual affected with Familial Hemophagocytic Lymphohistiocytosis (Shabrish_2021). These data do not allow any conclusion about variant significance. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication have been ascertained in the context of this evaluation (PMID: 33746956). ClinVar contains an entry for this variant (Variation ID: 533099). Based on the evidence outlined above, the variant was classified as uncertain significance. -
Familial hemophagocytic lymphohistiocytosis 3 Uncertain:1
This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 286 of the UNC13D protein (p.Arg286Trp). This variant is present in population databases (rs142618705, gnomAD 0.01%). This missense change has been observed in individual(s) with FHL3 (PMID: 33746956). ClinVar contains an entry for this variant (Variation ID: 533099). An algorithm developed to predict the effect of missense changes on protein structure and function outputs the following: PolyPhen-2: "Possibly Damaging". The tryptophan amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
