The variant received 0 classification points (ACGS-UK Somatic Oncogenicity v2025).

The NM_021625.5(TRPV4):c.616C>T (p.Arg206Cys) variant causes a missense change involving the alteration of a non-conserved nucleotide. The gene TRPV4 is a known oncogene (CancerMine: 6 oncogene, 3 driver citations). The gene TRPV4 is a cancer driver gene (CancerMine: 6 oncogene, 3 driver citations). The variant allele was found at a cumulative frequency of 0.0000445 (AC=65) in the gnomAD database across 1,461,830 control chromosomes (no homozygotes observed). The grpmax filtering allele frequency (95% CI) is 0.000154. Note: a gnomAD entry for this variant shows a statistical allele-bias signature, consistent with mosaic/somatic contamination (e.g. age-related clonal hematopoiesis) rather than true inherited population frequency — this frequency should not be read as evidence of a common, benign germline variant. Splicing prediction tools (SpliceAI) predict no significant impact on normal splicing. Variant has been reported in ClinVar as Benign/Likely Benign (no review stars). Other variants at the same amino acid position have been reported in ClinVar (not pathogenic): p.R206H: Uncertain_significance (ClinVar VariationId 684495, 2 stars); p.R206L: Uncertain_significance (ClinVar VariationId 1320389, 2 stars) The variant has been observed in cBioPortal in 3 samples across 3 studies and 3 cancer types; somatic enrichment level: SUPPORTING_LOW (Observed in a small number of cBioPortal cases.). This exact variant is curated in the UniProt human variants database as Uncertain Significance, associated with Nevi and Melanomas; it is also listed as a COSMIC curated somatic variant.

Genes affected
TRPV4 (HGNC:18083): (transient receptor potential cation channel subfamily V member 4) This gene encodes a member of the OSM9-like transient receptor potential channel (OTRPC) subfamily in the transient receptor potential (TRP) superfamily of ion channels. The encoded protein is a Ca2+-permeable, nonselective cation channel that is thought to be involved in the regulation of systemic osmotic pressure. Mutations in this gene are the cause of spondylometaphyseal and metatropic dysplasia and hereditary motor and sensory neuropathy type IIC. Multiple transcript variants encoding different isoforms have been found for this gene. [provided by RefSeq, Apr 2010]
