Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4e-05 in 251240 control chromosomes (gnomAD). c.202C>G has been reported in the literature in multiple individuals affected with Homocystinuria Due To Methylene Tetrahydrofolate Reductase Deficiency and was shown to segregate with the disease (examples: Rommer_2017 and Kaur_2020). At least one publication reports experimental evidence evaluating an impact on protein function. Burda_2017 demonstrated that this variant increases the affinity of the protein to NADPH and reduced the MTHFR activity. One clinical diagnostic laboratory has submitted clinical-significance assessments for this variant to ClinVar after 2014 and classified the variant as pathogenic. In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 187869). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on MTHFR protein function.
