The p.N116S variant (also known as c.347A>G), located in coding exon 3 of the MPZ gene, results from an A to G substitution at nucleotide position 347. The asparagine at codon 116 is replaced by serine, an amino acid with highly similar properties. This alteration was detected in the heterozygous state in a patient with Charcot-Marie-Tooth disease (Kleffner I et al. J Neurol, 2010 Nov;257:1864-8). Other alterations at this amino acid position (p.N116Y and p.N116H) have also been reported in patients with Charcot-Marie-Tooth disease (Takashima H et al. Genet Med;3:335-42; Gentile L et al. Neurol Sci, 2020 May;41:1239-1243). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

This sequence change replaces asparagine, which is neutral and polar, with serine, which is neutral and polar, at codon 116 of the MPZ protein (p.Asn116Ser). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with axonal neuropathy and/or Charcot-Marie-Tooth disease (PMID: 20556410, 36203352). ClinVar contains an entry for this variant (Variation ID: 41026). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt MPZ protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
