This TNNT2 variant was found in compound heterozygosity with one another TNNT2 variant in a young patient with severe dilated cardiomyopathy. The patient also harbours a pathogenic variant in TTN gene. The combination of these three variants explains this severe phenotype in a such young patient. -

This missense variant replaces arginine with histidine at codon 151 of the TNNT2 protein. This variant is found within a highly conserved region of the tropomyosin binding domain. Missense variants in this region have been shown to be significantly overrepresented in individuals with affected with hypertrophic cardiomyopathy (PMID: 30696458). Computational prediction tools indicate that this variant has a deleterious impact on protein structure and function. To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with left ventricular noncompaction cardiomyopathy (PMID: 20530761). This variant has been identified in 1/248554 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

This sequence change replaces arginine, which is basic and polar, with histidine, which is basic and polar, at codon 151 of the TNNT2 protein (p.Arg151His). The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This missense change has been observed in individual(s) with dilated cardiomyopathy and/or left ventricular noncompaction (PMID: 20530761, 37652022). ClinVar contains an entry for this variant (Variation ID: 437410). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt TNNT2 protein function with a negative predictive value of 80%. This variant disrupts the p.Arg151 amino acid residue in TNNT2. Other variant(s) that disrupt this residue have been observed in individuals with TNNT2-related conditions (PMID: 32969603), which suggests that this may be a clinically significant amino acid residue. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
