This sequence change replaces histidine with arginine at codon 210 of the ALPL protein (p.His210Arg). The histidine residue is moderately conserved and there is a small physicochemical difference between histidine and arginine. This variant is present in population databases (rs748031071, ExAC 0.003%). This variant has been observed in individual(s) with hypophosphatasia (PMID: 25731960). ClinVar contains an entry for this variant (Variation ID: 556859). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt ALPL protein function. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively.
