The NM_001048174.2(MUTYH):c.1130C>T (p.Pro377Leu) variant causes a missense change involving the alteration of a non-conserved nucleotide. The gene MUTYH is a cancer driver gene (CancerMine: 2 driver citations). The variant allele was found at a cumulative frequency of 0.0000682 (AC=110) in the gnomAD database across 1,614,020 control chromosomes (no homozygotes observed). The grpmax filtering allele frequency (95% CI) is 0.0000745. In-silico predictor (REVEL) classifies this variant as likely damaging/oncogenic. Splicing prediction tools (SpliceAI) predict no significant impact on normal splicing. Variant has been reported in ClinVar as Pathogenic/Likely Pathogenic (★★). ClinVar reports functional evidence for this variant: "SCV000186969: Functional analysis of this alteration has revealed compromised adenine glycosylase activity that was 30-40% of the activity of the wildtype enzyme (Kundu S et al. DNA Repair (Amst.) 2009 Dec;8:1400-10)." and additional evidence is available in ClinVar. A different missense at the same amino acid position has been reported as Pathogenic/Likely Pathogenic in ClinVar: p.P377R: Likely_pathogenic (ClinVar VariationId 3914955, 1 star) Other variants at the same amino acid position have been reported in ClinVar (not pathogenic): p.P377= (synonymous): Likely_benign (ClinVar VariationId 232703, 2 stars); p.P377= (synonymous): Likely_benign (ClinVar VariationId 2065560, 2 stars); p.P377= (synonymous): Benign/Likely_benign (ClinVar VariationId 414158, 2 stars); p.P377S: Uncertain_significance (ClinVar VariationId 5298, 1 star); p.P377dup: Uncertain_significance (ClinVar VariationId 464679, 2 stars) The variant has been observed in cBioPortal in 4 samples across 2 studies and 2 cancer types; somatic enrichment level: SUPPORTING_LOW (Observed in a small number of cBioPortal cases.). This exact variant is curated in the UniProt human variants database as Uncertain Significance.

Genes affected
MUTYH (HGNC:7527): (mutY DNA glycosylase) This gene encodes a DNA glycosylase involved in oxidative DNA damage repair. The enzyme excises adenine bases from the DNA backbone at sites where adenine is inappropriately paired with guanine, cytosine, or 8-oxo-7,8-dihydroguanine, a major oxidatively damaged DNA lesion. The protein is localized to the nucleus and mitochondria. This gene product is thought to play a role in signaling apoptosis by the introduction of single-strand breaks following oxidative damage. Mutations in this gene result in heritable predisposition to colorectal cancer, termed MUTYH-associated polyposis (MAP). Multiple transcript variants encoding different isoforms have been found for this gene. [provided by RefSeq, Apr 2017]

Classification according to ACGS-UK Somatic Oncogenicity v2025

Classification was made for transcript:

Our verdict: Uncertain_significance. The variant received 2 points.

O3Absent or very rare in population
Very rare in gnomAD (popmax AF <0.01%) — O3 supporting; GnomAD effective popmax AF = 0.000075 — very rare (O3 supporting [+1])
O6Computational evidence supports deleterious effect
Computational evidence supports a deleterious/oncogenic effect; Missense variant — primary computational scorer supports an oncogenic/deleterious effect
