In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0"). ClinVar contains an entry for this variant (Variation ID: 47900). This missense change has been observed in individual(s) with dilated cardiomyopathy (PMID: 27532257). This variant is present in population databases (rs374000722, gnomAD 0.006%). This sequence change replaces threonine, which is neutral and polar, with alanine, which is neutral and non-polar, at codon 666 of the NEXN protein (p.Thr666Ala). -

Variant classified as Uncertain Significance - Favor Benign. The Thr666Ala varia nt in NEXN has been identified in 1/6546 European American chromosomes from a br oad population by the NHLBI Exome Sequencing Project (http://evs.gs.washington.e du/EVS/); this may represent a presymptomatic individual. Our laboratory has ide ntified this variant in one individual with early onset DCM who carried a second variant of unknown significance. While one parent had DCM, both variants were i nherited from the unaffected parent, suggesting that they are not the primary ca use of disease in this family. Computational analyses (biochemical amino acid pr operties, conservation, AlignGVGD, PolyPhen2, and SIFT) do not provide strong su pport for or against an impact to the normal function of the protein. In summary , additional information is needed to fully assess the clinical significance of the Thr666Ala variant. -

The c.1996A>G (p.T666A) alteration is located in exon 13 (coding exon 12) of the NEXN gene. This alteration results from a A to G substitution at nucleotide position 1996, causing the threonine (T) at amino acid position 666 to be replaced by an alanine (A). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear. -

The NEXN c.1996A>G variant is predicted to result in the amino acid substitution p.Thr666Ala. This variant has been reported in individuals with dilated cardiomyopathy (Haas et al. 2015. PubMed ID: 25163546; Table S1B, Walsh et al. 2017. PubMed ID: 27532257). This variant is reported in 0.0055% of alleles in individuals of European (Non-Finnish) descent in gnomAD. At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -
