The R518C variant was not observed in approximately 2,200 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The R518C variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. The R518C variant results in a gain of Cysteine residue, which may impact disulfide bonding. This substitution occurs at a position that is conserved across species. In silico analysis is inconsistent in its predictions as to whether or not the variant is damaging to the protein structure/function. Mutations in nearby residues have not been reported in association with cardiomyopathy, indicating this region of the protein may tolerate change. The variant is found in CARDIOMYOPATHY panel(s). - |

This variant has not been reported in the literature. This variant lies outside of exon 9, which contains a region that appears to be especially susceptible to pathogenic variation. "Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0")." The arginine at codon 518 is highly conserved across species, as are neighboring amino acids. Per ClinVar, no other variants have been reported in association with disease at this codon or in and nearby codons. The variant was reported online in 3 of 91,080 individuals in the Genome Aggregation Consortium Dataset (gnomAD; http://gnomad.broadinstitute.org/), which currently includes variant calls on >140,000 unrelated individuals of African, Asian, European, Ashkenazi, Latino descent. Specifically, the variant was observed in 2 of 36,430 individuals of European descent (MAF=0.0027%) and 1 of 11,402 individuals of South Asian descent. The phenotype of those individuals is not publicly available. The dataset is comprised of multiple cohorts, some of which were recruited from the general population, others were enriched for common cardiovascular disease. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.1552C>T has been reported in the literature in individual(s) affected with Cardiomyopathy (e.g. Parikh_2019). These report(s) do not provide unequivocal conclusions about association of the variant with Cardiomyopathy. A co-occurrence with a pathogenic variant has been reported (MYBPC3 c.3330+2T>A; Internal testing). To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Two ClinVar submitters (evaluation after 2014) cite the variant as uncertain significance. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on RBM20 protein function. This variant was reported in one individual from a dilated cardiomyopathy (DCM) cohort (Saito T et al. ESC Heart Fail, 2021 Dec;8:5178-5191). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. - |
