RET (HGNC:9967): (ret proto-oncogene) This gene encodes a transmembrane receptor and member of the tyrosine protein kinase family of proteins. Binding of ligands such as GDNF (glial cell-line derived neurotrophic factor) and other related proteins to the encoded receptor stimulates receptor dimerization and activation of downstream signaling pathways that play a role in cell differentiation, growth, migration and survival. The encoded receptor is important in development of the nervous system, and the development of organs and tissues derived from the neural crest. This proto-oncogene can undergo oncogenic activation through both cytogenetic rearrangement and activating point mutations. Mutations in this gene are associated with Hirschsprung disease and central hypoventilation syndrome and have been identified in patients with renal agenesis. [provided by RefSeq, Sep 2017]

This missense variant replaces threonine with alanine at codon 278 of the RET protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with paraganglioma (PMID: 34750850). This variant has been identified in 7/245974 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

The p.T278A variant (also known as c.832A>G), located in coding exon 4 of the RET gene, results from an A to G substitution at nucleotide position 832. The threonine at codon 278 is replaced by alanine, an amino acid with similar properties. This alteration was reported in several Chinese or Vietnamese probands with Hirschsprung disease (So MT et al. PLoS ONE 2011 Dec;6(12):e28986; Tang CS et al. Gastroenterology. 2018 12;155:1908-1922.e5). This alteration was identified in 0/245 Chinese patients with MEN2, medullary thyroid carcinoma, and/ or pheochromocytoma and 1/493 of their unaffected relatives (Qi XP et al. BMC Cancer. 2021 Apr;21:369). This variant has been detected in multiple individuals with no reported features of multiple endocrine neoplasia type 2 (Ambry internal data). Based on data from gnomAD, the frequency for this variant is above the maximum credible frequency for a MEN2-causing variant in this gene based on internally established thresholds (Karczewski et al. Nature. 2020 May;581(7809):434-443; Whiffin et al. Genet Med. 2017 10;19:1151-1158). This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the supporting evidence, the association of this alteration with Hirschsprung disease is unknown; however, the association of this alteration with MEN2 is unlikely. -
