RET (HGNC:9967): (ret proto-oncogene) This gene encodes a transmembrane receptor and member of the tyrosine protein kinase family of proteins. Binding of ligands such as GDNF (glial cell-line derived neurotrophic factor) and other related proteins to the encoded receptor stimulates receptor dimerization and activation of downstream signaling pathways that play a role in cell differentiation, growth, migration and survival. The encoded receptor is important in development of the nervous system, and the development of organs and tissues derived from the neural crest. This proto-oncogene can undergo oncogenic activation through both cytogenetic rearrangement and activating point mutations. This missense variant replaces valine with phenylalanine at codon 704 of the RET protein. Computational prediction is inconclusive regarding the impact of this variant on protein structure and function (internally defined REVEL score threshold 0.5 < inconclusive < 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with hyperparathyroidism (PMID: 28946813), and it also has been reported as a heterozygous carrier affected with congenital anomalies of kidney and urinary tract (PMID: 21900877, 24429398). This variant has been identified in 5/280144 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

The p.V704F variant (also known as c.2110G>T), located in coding exon 11 of the RET gene, results from a G to T substitution at nucleotide position 2110. The valine at codon 704 is replaced by phenylalanine, an amino acid with highly similar properties. This alteration has been reported in individuals with congenital abnormalities of the kidney and urinary tract (CAKUT) (Hwang DY et al. Kidney Int. 2014 Jun;85:1429-33; Saisawat P et al. Kidney Int. 2012 Jan;81:196-200), as well as in an individual with hyperparathyroidism (Lebeault M et al. Thyroid 2017 Dec;27:1511-1522). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
