Genes affected
RET (HGNC:9967): (ret proto-oncogene) This gene encodes a transmembrane receptor and member of the tyrosine protein kinase family of proteins. Binding of ligands such as GDNF (glial cell-line derived neurotrophic factor) and other related proteins to the encoded receptor stimulates receptor dimerization and activation of downstream signaling pathways that play a role in cell differentiation, growth, migration and survival. The encoded receptor is important in development of the nervous system, and the development of organs and tissues derived from the neural crest. This proto-oncogene can undergo oncogenic activation through both cytogenetic rearrangement and activating point mutations. Mutations in this gene are associated with Hirschsprung disease and central hypoventilation syndrome and have been identified in patients with renal agenesis. [provided by RefSeq, Sep 2017]

Uncertain significance, criteria provided, single submitter | clinical testing | All of Us Research Program, National Institutes of Health | Jun 08, 2023 | This missense variant replaces asparagine with serine at codon 783 of the RET protein. Computational prediction is inconclusive regarding the impact of this variant on protein structure and function (internally defined REVEL score threshold 0.5 < inconclusive < 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with medullary thyroid cancer (PMID: 28946813) and an individual affected with isolated primary hyperparathyroidism (PMID: 33340421). This variant has been identified in 1/251344 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. - |
Uncertain significance, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Nov 03, 2023 | This sequence change replaces asparagine, which is neutral and polar, with serine, which is neutral and polar, at codon 783 of the RET protein (p.Asn783Ser). This variant is present in population databases (rs587778656, gnomAD 0.0009%). This missense change has been observed in individual(s) with Hirschsrpung disease and medullary thyroid cancer (PMID: 22174939, 28946813). ClinVar contains an entry for this variant (Variation ID: 549812). Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Not Available"; PolyPhen-2: "Benign"; Align-GVGD: "Not Available". The serine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. - |

Uncertain significance, criteria provided, single submitter | clinical testing | GeneDx | Apr 01, 2022 | Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports a deleterious effect on splicing; In silico analysis supports that this missense variant does not alter protein structure/function; Identified in a patient with Hirschsprung disease and shown to have been maternally inherited in published literature (So et al., 2011); Identified in a patient with isolated primary hyperparathyroidism and classified as a variant of uncertain significance in published literature (Fussey et al., 2021); This variant is associated with the following publications: (PMID: 14633923, 28946813, 22174939, 33340421) - |
Hereditary cancer-predisposing syndrome Uncertain:1
Uncertain significance, criteria provided, single submitter | clinical testing | Ambry Genetics | Feb 13, 2024 | The p.N783S variant (also known as c.2348A>G), located in coding exon 13 of the RET gene, results from an A to G substitution at nucleotide position 2348. The asparagine at codon 783 is replaced by serine, an amino acid with highly similar properties. This alteration has been identified in multiple Chinese patients diagnosed with Hirschsprung disease as well as one French patient diagnosed with medullary thyroid cancer at age 57 (So MT et al. PLoS ONE. 2011 Dec;6:e28986; Tang CS et al. Gastroenterology. 2018 12;155:1908-1922.e5; Lebeault M et al. Thyroid. 2017 12;27:1511-1522). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Based on the available evidence, the clinical significance of this alteration remains unclear. - |
