Variant summary
Our verdict is Uncertain significance. The variant received 5 ACMG points: 5P and 0B. PP3_StrongPP5
The NM_020549.5(CHAT):c.1321G>A(p.Glu441Lys) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.00000558 in 1,613,860 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. 13/22 in silico tools predict a damaging outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars).

This sequence change replaces glutamic acid, which is acidic and polar, with lysine, which is basic and polar, at codon 441 of the CHAT protein (p.Glu441Lys). The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This missense change has been observed in individual(s) with congenital myasthenic syndrome (PMID: 11172068). ClinVar contains an entry for this variant (Variation ID: 17507). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt CHAT protein function with a positive predictive value of 95%. Experimental studies have shown that this missense change affects CHAT function (PMID: 11172068). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
Congenital myasthenic syndrome Pathogenic:1
Variant summary: CHAT c.1321G>A (p.Glu441Lys) results in a conservative amino acid change located in the CoA-dependent acyltransferases of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 251050 control chromosomes. c.1321G>A has been reported in the literature in individuals affected with Congenital Myasthenic Syndrome (Ohno_2001, Abitht_2012). These data indicate that the variant may be associated with disease. At least one publication reports experimental evidence evaluating an impact on protein function, showing the variant resulted in low expression and absent catalytic activity (Ohno_2001). The following publications have been ascertained in the context of this evaluation (PMID: 22678886, 11172068). ClinVar contains an entry for this variant (Variation ID: 17507). Based on the evidence outlined above, the variant was classified as likely pathogenic. -
