Variant summary
Our verdict is Likely pathogenic. The variant received 7 ACMG points: 7P and 0B. PM2PM5PP3_ModeratePP5
The NM_000051.4(ATM):āc.1234T>Cā(p.Trp412Arg) variant causes a missense, splice region change. The variant allele was found at a frequency of 0.00000248 in 1,613,778 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. 2/3 splice prediction tools predict no significant impact on normal splicing. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. W412C) has been classified as Uncertain significance.

Ataxia-telangiectasia syndrome Pathogenic:1Uncertain:1
This sequence change replaces tryptophan, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 412 of the ATM protein (p.Trp412Arg). This variant is present in population databases (rs587779812, gnomAD 0.0009%). This missense change has been observed in individual(s) with ataxia telangiectasia (internal data). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 127333). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change affects ATM function (PMID: 25040471). RNA analysis performed to evaluate the impact of this missense change on mRNA splicing indicates it does not significantly alter splicing (internal data). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -

Hereditary cancer-predisposing syndrome Uncertain:2
This missense variant replaces tryptophan with arginine at codon 412 of the ATM protein. Computational prediction is inconclusive regarding the impact of this variant on protein structure and function. A functional study showed that this variant reduced protein levels and kinase activity (PMID: 25040471). This variant has been reported in an individual affected with breast cancer (PMID: 33471991). This variant has been identified in 1/251052 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.W412R variant (also known as c.1234T>C), located in coding exon 8 of the ATM gene, results from a T to C substitution at nucleotide position 1234. The tryptophan at codon 412 is replaced by arginine, an amino acid with dissimilar properties. In one study, this variant was shown to result in reduced ATM protein levels and residual kinase activity in patient cells (Taylor AM et al. Clin. Genet. 2015 Mar; 87(3):199-208). This variant has been confirmed in trans with an ATM pathogenic variant in an individual with clinical features of ataxia telangiectasia (Ambry internal data). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -
not provided Uncertain:1
Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Published functional studies show reduced protein expression, but some retained irradiation-induced kinase activity, the clinical significance of which is unclear (PMID: 25040471); This variant is associated with the following publications: (PMID: 25040471, 33471991) -
