The ATM p.Arg451Cys variant was not identified in the literature nor was it identified in the GeneInsight-COGR, MutDB, LOVD 3.0, ATM-LOVD, databases. The variant was identified in the following databases: dbSNP (ID: rs201719927) as With Uncertain significance allele, ClinVar (classified as uncertain significance by Ambry Genetics, Invitae, GeneDx), Cosmic (classified as Pathogenic (score 0.96)). The variant was identified in control databases in 30 of 246126 chromosomes at a frequency of 0.000122 increasing the likelihood this could be a low frequency variant (Genome Aggregation Consortium Feb 27, 2017). The p.Arg451 residue is conserved in mammals but not in more distantly related organisms, and computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) suggest that the variant may impact the protein; however, this information is not predictive enough to assume pathogenicity. The variant occurs outside of the splicing consensus sequence and 1 of 5 in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer, HumanSpliceFinder) predict a greater than 10% difference in splicing; this is not very predictive of pathogenicity. In summary, based on the above information, the clinical significance of this variant cannot be determined with certainty at this time. This variant is classified as a variant of uncertain significance.
In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; This variant is associated with the following publications: (PMID: 24356096, 26689913, 28652578, 30287823, 31719806, 27104957, 33436325, 26911350, 30093976, 31742824, 28779002, 32973888, 36627197, 36243179, 33471991)
Hereditary cancer-predisposing syndrome Uncertain:1Benign:2
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity.

Ataxia-telangiectasia syndrome;C0346153:Familial cancer of breast Uncertain:1
ATM-related disorder Uncertain:1
The ATM c.1351C>T variant is predicted to result in the amino acid substitution p.Arg451Cys. This variant has been reported in cases and controls from a breast cancer cohort study (Supp. Data, Momozawa et al. 2018. PubMed ID: 30287823), an individual with a personal and family history of breast cancer (Table S2, Chan et al. 2018. PubMed ID: 30093976), and (Table S4, Karlsson et al. 2021. PubMed ID: 33436325). This variant is reported in 0.082% of alleles in individuals of South Asian descent in gnomAD (http://gnomad.broadinstitute.org/variant/11-108121543-C-T). At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence.
not specified Benign:1
Variant summary: ATM c.1351C>T (p.Arg451Cys) results in a non-conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 0.00013 in 251354 control chromosomes, predominantly at a frequency of 0.00082 within the South Asian subpopulation in the gnomAD database. c.1351C>T has been reported in the literature in individuals affected with breast cancer, prostate cancer, or biliary tract cancer, as well as unaffected controls (e.g. Momozawa_2018, Chan_2018, Karlsson_2021, Okawa_2023). These reports do not provide unequivocal conclusions about association of the variant with Breast Cancer. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 30287823, 30093976, 33436325, 36243179). ClinVar contains an entry for this variant (Variation ID: 185437). Based on the evidence outlined above, the variant was classified as likely benign.
Familial cancer of breast Benign:1
This variant is considered likely benign. This variant is strongly associated with less severe personal and family histories of cancer, typical for individuals without pathogenic variants in this gene [PMID: 25085752].
