The arginine at codon 451 is replaced by histidine, an amino acid with highly similar properties. This alteration was observed in 3/7051 unselected female breast cancer patients and was observed with an allele frequency of 0.00018 in 11241 female controls of Japanese ancestry. In addition, it was not observed in unselected male breast cancer patients and was observed in 5/12490 male controls of Japanese ancestry (Momozawa Y et al. Nat Commun, 2018 10;9:4083). This alteration has been reported with a carrier frequency of 0.00052 in 7636 unselected prostate cancer patients and 0.65 in 12366 male controls of Japanese ancestry (Momozawa Y et al. J Natl Cancer Inst, 2020 04;112:369-376). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. - |

Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. In a large international case-control study, this variant was absent in 60466 breast cancer cases and reported in 1/53461 controls (PMID: 33471991). In a separate case-control study conducted in Japan, this variant was reported in 3/7051 women affected with breast cancer, 2/11241 unaffected women, 0/53 men affected with breast cancer, and 8/12490 unaffected men (PMID: 30287823). In a third study the variant was reported in 4/7636 prostate cancer cases & 8/12366 unaffected controls (PMID: 31214711). This variant has been identified in 2/251368 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. - |
