Uncertain significance, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Nov 01, 2022 | This sequence change replaces arginine, which is basic and polar, with cysteine, which is neutral and slightly polar, at codon 785 of the ATM protein (p.Arg785Cys). This variant is present in population databases (rs587778065, gnomAD 0.006%). This missense change has been observed in individual(s) with ataxia-telangiectasia (PMID: 9887333). ClinVar contains an entry for this variant (Variation ID: 133605). Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Tolerated"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0". The cysteine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. - |

Uncertain significance, criteria provided, single submitter | clinical testing | Quest Diagnostics Nichols Institute San Juan Capistrano | Nov 15, 2024 | The ATM c.2353C>T (p.Arg785Cys) variant has been reported in individuals with Ataxia telangiectasia (AT) (PMID: 9887333 (1999)), acute pancreatitis (PMID: 35171259 (2022)), and breast cancer (PMID: 20305132 (2010)). This variant has also been identified in reportedly healthy individuals (PMID: 24728327 (2014), 33471991 (2021), see also LOVD (http://databases.lovd.nl/shared)). The frequency of this variant in the general population, 0.000012 (3/251292 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded conflicting predictions that this variant is deleterious or benign. Based on the available information, we are unable to determine the clinical significance of this variant. - |

Hereditary cancer-predisposing syndrome Uncertain:2
Uncertain significance, criteria provided, single submitter | clinical testing | Ambry Genetics | Jun 29, 2023 | The p.R785C variant (also known as c.2353C>T), located in coding exon 14 of the ATM gene, results from a C to T substitution at nucleotide position 2353. The arginine at codon 785 is replaced by cysteine, an amino acid with highly dissimilar properties. This alteration was identified in the heterozygous state in an individual with ataxia telangiectasia; a second alteration was not identified (Sandoval N et al. Hum. Mol. Genet. 1999 Jan;8(1):69-79). In addition, this variant was identified in a genome-wide scan of 681 healthy individuals from diverse backgrounds with an allele frequency of 0.0007 in all populations, and 0.0116 in the African population (Bodian DL et al. PLoS ONE. 2014 Apr;9(4):e94554). This amino acid position is poorly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. - |
Uncertain significance, criteria provided, single submitter | clinical testing | Color Diagnostics, LLC DBA Color Health | Oct 02, 2023 | This missense variant replaces arginine with cysteine at codon 785 of the ATM protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in the heterozygous state in an individual affected with ataxia telangiectasia (PMID: 9887333). This variant has been detected in a breast cancer case-control meta-analysis in 0/60466 cases and 2/53461 unaffected individuals (PMID: 33471991; Leiden Open Variation Database DB-ID ATM_000092). This variant has also been identified in 3/251292 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. - |

Uncertain significance, criteria provided, single submitter | clinical testing | Women's Health and Genetics/Laboratory Corporation of America, LabCorp | Sep 12, 2023 | Variant summary: ATM c.2353C>T (p.Arg785Cys) results in a non-conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 1.2e-05 in 251292 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.2353C>T has been reported in the literature in individuals affected with Ataxia Telangiectasia, Breast Cancer, and Acute Pancreatitis (example, Sandoval_1999, Yin_2022, Bernstein_2010), without strong evidence of causality. This variant has also been reported in unaffected control cohorts from large case-control studies of Breast cancer (PMID: 33471991, 24728327). These report(s) do not provide unequivocal conclusions about association of the variant with Ataxia-Telangiectasia or Breast Cancer. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 20305132, 9887333, 35171259). Seven submitters have cited clinical-significance assessments for this variant to ClinVar after 2014. All submitters classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. - |
