This protein is an important cell cycle checkpoint kinase that phosphorylates; thus, it functions as a regulator of a wide variety of downstream proteins, including tumor suppressor proteins p53 and BRCA1, checkpoint kinase CHK2, checkpoint proteins RAD17 and RAD9, and DNA repair protein NBS1. This protein and the closely related kinase ATR are thought to be master controllers of cell cycle checkpoint signaling pathways that are required for cell response to DNA damage and for genome stability. BS1, BP4 c.6998C>A, located in exon 48 of the ATM gene, is predicted to result in the substitution of threonine by lysine at codon 2333, p.(Thr2333Lys). This variant is found in 28/23606 at a filter allele frequency of 0.084% in the gnomAD v2.1.1 database (African non-cancer data set)(BS1). The SpliceAI algorithm predicts no significant impact on splicing and the REVEL meta-predictor score for this variant (0.116) suggests that it does not affect the protein function (BP4). To our knowledge, functional studies have not been reported for this variant. In addition, it has been reported in ClinVar (10x likely benign, 2x uncertain significance) and LOVD (1x VUS, 2x not provided) databases. Based on currently available information, the variant c.6998C>A is classified as a likely benign variant according to ClinGen-ATM Guidelines version 1.1. -

Variant summary: ATM c.6998C>A (p.Thr2333Lys) results in a non-conservative amino acid change located in the PIK-related kinase, FAT domain (IPR003151) of the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 9.6e-05 in 251024 control chromosomes, predominantly at a frequency of 0.0014 within the African or African-American subpopulation in the gnomAD database. The observed variant frequency within African or African-American control individuals in the gnomAD database is approximately 1.4 fold of the estimated maximal expected allele frequency for a pathogenic variant in ATM causing Breast Cancer phenotype (0.001), strongly suggesting that the variant is a benign polymorphism found primarily in populations of African or African-American origin. c.6998C>A has been reported in the literature in individuals affected with breast cancer, hepatoblastoma, colon adenocarcinoma, or primary ovarian insufficiency, all without strong evidence for causality (e.g., Tung_2015, Young_2016, Aguiar_2022, Heddar_2022, Piha-Paul_2024). Additionally, this variant has also been reported in at-least one patient with Ataxia Telangiectasia (AT) who was compound heterozygous for two other deletrious variants in the ATM gene (Micol_2011). Although the phase of this variant relative to either of the deleterious variants was not provided, this finding supports a benign impact for this variant. Taken together, these reports do not provide unequivocal conclusions about association of the variant with Breast Cancer. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 25186627, 26787654, 21665257, 35495172, 36099812, 39085400). ClinVar contains an entry for this variant (Variation ID: 127434). Based on the evidence outlined above, the variant was classified as likely benign. -
