The encoded preproprotein is proteolytically processed to generate a protein that binds to the parathyroid hormone/parathyroid hormone-related peptide receptor and regulates blood calcium and phosphate levels. Excess production of the encoded protein, known as hyperparathyroidism, can result in hypercalcemia and kidney stones. On the other hand, defective processing of the encoded protein may lead to hypoparathyroidism, which can result in hypocalcemia and numbness. Likely pathogenic, criteria provided, single submitter | clinical testing | GeneDx | Aug 31, 2016 | The R56C variant in the PTH gene has been reported previously in the homozygous state in three siblings with biochemical findings consistent with hypoparathyroidism, one of whom exhibited clinical features including muscle cramps and seizures (Lee et al., 2015). Functional studies demonstrate that the R56C variant results in decreased binding to the PTH1R receptor (Lee et al., 2015). This variant was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The R56C variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position that is not conserved across species. We interpret R56C as a likely pathogenic variant. - |
