Variant summary

Our verdict is Pathogenic. The variant received 18 ACMG points: 18P and 0B. PS3PM2PP3_StrongPP5_Very_Strong

The NM_181486.4(TBX5):c.668C>T(p.Thr223Met) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. 14/22 in silico tools predict a damaging outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★★). ClinVar reports functional evidence for this variant: "SCV000616887: Published functional studies demonstrate that T223M interferes with the normal DNA-binding of the TBX5 protein (Stirnimann et al., 2010" and additional evidence is available in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. T223P) has been classified as Uncertain significance.

Genes affected
TBX5 (HGNC:11604): (T-box transcription factor 5) This gene is a member of a phylogenetically conserved family of genes that share a common DNA-binding domain, the T-box. T-box genes encode transcription factors involved in the regulation of developmental processes. This gene is closely linked to related family member T-box 3 (ulnar mammary syndrome) on human chromosome 12. The encoded protein may play a role in heart development and specification of limb identity. Mutations in this gene have been associated with Holt-Oram syndrome, a developmental disorder affecting the heart and upper limbs. Several transcript variants encoding different isoforms have been described for this gene. [provided by RefSeq, Jul 2008]

ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 18 ACMG points.

PS3
PS3 evidence extracted from ClinVar submissions: SCV000616887: Published functional studies demonstrate that T223M interferes with the normal DNA-binding of the TBX5 protein (Stirnimann et al., 2010; Darwich et al., 2017; Markunas et al., 2021)
PM2
Very rare variant in population databases, with high coverage;
PP3
MetaRNN computational evidence supports a deleterious effect, 0.95
PP5
Variant 12-114385563-G-A is Pathogenic according to our data. Variant chr12-114385563-G-A is described in ClinVar as Pathogenic. ClinVar VariationId is 449105.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.
