Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 1.6e-05 in 251308 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.1941T>G has been reported in the literature in individuals affected with hypertrophic and arrhythmogenic cardiomyopathy as well as ventricular tachycardia (e.g., Lopes_2013, Vischer_2019, Dries_2021, Protonotarios_2022), however without strong evidence for causality (e.g., lack of co-segregation data). These report(s) do not provide unequivocal conclusions about association of the variant with Arrhythmogenic Right Ventricular Dysplasia/Cardiomyopathy. A co-occurrence with another pathogenic variant has been reported (PKP2 c.2197_2202delinsG, p.His733AlafsX8; Vischer_2019), providing supporting evidence for a benign role. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The cysteine at codon 647 is replaced by tryptophan, an amino acid with highly dissimilar properties. This variant has been reported in a hypertrophic cardiomyopathy cohort; however, clinical details were limited, and it occurred in conjunction with variants in other cardiac-related genes (Lopes LR et al. J. Med. Genet., 2013 Apr;50:228-39 (reported as p.C603W)). This variant was also reported in a child with arrhythmogenic right ventricular cardiomyopathy (ARVC) and heart failure, who had an additional PKP2 frameshift variant also detected (Vischer AS et al. Int J Cardiol, 2019 07;286:99-103). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. - |
