Variant summary
Our verdict is Uncertain significance. The variant received 4 ACMG points: 4P and 0B. PM2PP2PP3
The NM_001330260.2(SCN8A):c.3230A>G(p.Tyr1077Cys) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.00000248 in 1,613,836 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. Y1077H) has been classified as Uncertain significance.

Developmental and epileptic encephalopathy Uncertain:1
This variant has not been reported in the literature in individuals with SCN8A-related disease. This variant is present in population databases (rs746302474, ExAC 0.003%). This sequence change replaces tyrosine with cysteine at codon 1077 of the SCN8A protein (p.Tyr1077Cys). The tyrosine residue is highly conserved and there is a large physicochemical difference between tyrosine and cysteine. Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
