ACVRL1 (HGNC:175): (activin A receptor like type 1) This gene encodes a type I cell-surface receptor for the TGF-beta superfamily of ligands. It shares with other type I receptors a high degree of similarity in serine-threonine kinase subdomains, a glycine- and serine-rich region (called the GS domain) preceding the kinase domain, and a short C-terminal tail. The encoded protein, sometimes termed ALK1, shares similar domain structures with other closely related ALK or activin receptor-like kinase proteins that form a subfamily of receptor serine/threonine kinases. This sequence change replaces serine, which is neutral and polar, with phenylalanine, which is neutral and non-polar, at codon 284 of the ACVRL1 protein (p.Ser284Phe). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with hereditary hemorrhagic telangiectasia (PMID: 15712271; Invitae). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. The p.S284F variant (also known as c.851C>T), located in coding exon 6 of the ACVRL1 gene, results from a C to T substitution at nucleotide position 851. The serine at codon 284 is replaced by phenylalanine, an amino acid with highly dissimilar properties. This variant was identified in a family with epistaxis, telangiectasias, gastrointestinal bleeding, and hepatic shunting (Abdalla SA et al. Hum. Mutat., 2005 Mar;25:320-1). In our internal cohort, this variant was identified in an individual with a clinical diagnosis of hereditary hemorrhagic telangiectasia (Ambry internal data) This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the majority of available evidence to date, this variant is likely to be pathogenic. -
