This variant is expected to result in an absent or non-functional protein product. This variant has been reported in individuals with a personal or family history of breast or ovarian cancer (PMID: 15131399, 29446198, 36881271) and in an unaffected individual (PMID: 33471991; Leiden Open Variation Database DB-ID BRCA2_004785). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). Loss of BRCA2 function is a known mechanism of disease (clinicalgenome.org). This duplication of one nucleotide in BRCA2 is denoted c.5471dupA at the cDNA level and p.Asn1824LysfsX5 (N1824KfsX5) at the protein level. The normal sequence, with the base that is duplicated in brackets, is TAAAA[dupA]TGCA. The duplication causes a frameshift, which changes an Asparagine to a Lysine at codon 1824, and creates a premature stop codon at position 5 of the new reading frame. This variant is predicted to cause loss of normal protein function through either protein truncation or nonsense-mediated mRNA decay. BRCA2 c.5471dupA, previously reported as 5699insA using alternate nomenclature, has been reported in association with hereditary breast and/or ovarian cancer (Lubinski 2004). We consider this variant to be pathogenic. -
The BRCA2 p.Asn1824Lysfsx5 variant was identified by Lubinski (2004) in a family referred for assessment of multiple cancer types. The variant was also identified in dbSNP (ID: rs80359515) “with pathogenic allele” and the BIC database (1X with clinical importance). The p.Asn1824Lysfsx5 variant is predicted to cause a frameshift, which alters the protein's amino acid sequence beginning at codon 1824 and leads to a premature stop codon 5 codons downstream. This alteration is then predicted to result in a truncated or absent protein and loss of function. Loss of function variants of the BRCA2 gene are an established mechanism of disease in hereditary breast and ovarian cancer and is the type of variant expected to cause the disorder. In summary, based on the above information, this variant meets our laboratory’s criteria to be classified as pathogenic. -
This frameshift variant alters the translational reading frame of the BRCA2 mRNA and causes the premature termination of BRCA2 protein synthesis. The c.5471dupA pathogenic mutation, located in coding exon 10 of the BRCA2 gene, results from a duplication of A at nucleotide position 5471, causing a translational frameshift with a predicted alternate stop codon (p.N1824Kfs*5). This alteration has previously been described as a mutation in one family in published literature; however, no clinical data was provided (Lubinski J et al. Fam Cancer. 2004;3(1):1-10). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -
