Four of five in-silico tools predict a benign effect of the variant on protein function. The variant was absent in 244700 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.5649A>C in individuals affected with Hereditary Breast and Ovarian Cancer and no experimental evidence demonstrating its impact on protein function have been reported. Three ClinVar submissions from clinical diagnostic laboratories (evaluation after 2014) cite the variant as uncertain significance (2x) or likely benign (1x). Based on the evidence outlined above, the variant was classified as uncertain significance. - |

The variant was not identified in control databases NHLBI Exome Sequencing Project (Exome Variant Server), Exome Aggregation Consortium (ExAC) or the 1000 Genomes Project. The p.Lys1883 residue is not conserved in mammals and the variant amino acid Asn is present in macaque, increasing the likelihood that this variant does not have clinical significance. In addition, four out of five computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) do not suggest a high likelihood of impact to the protein; however, this information is not predictive enough to rule out pathogenicity. The variant was identified by our laboratory in 1 individual with breast cancer, co-occurring with a pathogenic BRCA2 mutation (p.Tyr1894X), increasing the likelihood that the p.Lys1883Asn variant does not have clinical significance. In summary, based on the above information, the clinical significance of this variant cannot be determined with certainty at this time although we would lean towards a more benign role for this variant.
